Taking Issue with Sulcus IOL Choices

Is the Sulcus IOL the Right Choice for You?

In the July 2018 Review article The IOL In the Sulcus, the author demonstrates that this single-piece angulated foldable acrylic IOL can be considered for implantation in patients with sulcus-fixated IOLs. However, the author also notes that the IOL types recommended for implantation in the sulcus are either one-piece PMMA or three-piece acrylic IOLs.

This study raises some important questions about the use of Sulcus IOLs. First, it is not clear why the author recommends one-piece PMMA IOLs for implantation in the sulcus. PMMA IOLs are known to have a higher risk of capsular phimosis and posterior capsule opacification than acrylic IOLs. Second, the author does not provide any evidence to support the use of three-piece acrylic IOLs in the sulcus. In fact, some studies have shown that three-piece acrylic IOLs may be more likely to dislocate than one-piece acrylic IOLs.

Given these concerns, it is important to carefully consider the use of Sulcus IOLs. Patients who are considering Sulcus IOLs should discuss the risks and benefits of this procedure with their doctor before making a decision.
